Larkin Community Hospital, INC ("Larkin Community Hospital," "we," "us," or "our") operates an ambulance coordination service for Assisted Living Facilities ("ALFs") and related parties across Miami-Dade County, Florida. This Privacy Policy explains how we collect, use, disclose, and safeguard information you provide to us through our website, contact forms, telephone, SMS, email, and the course of providing coordination services.
By using our website or services, contacting us, or submitting information through any form, you acknowledge that you have read and understood this Privacy Policy. If you do not agree with this Policy, please do not use our website or submit information to us.
Important context. Larkin Community Hospital, INC provides coordination services, not direct medical transport. We are not a HIPAA-covered entity by default, but we treat any protected health information (PHI) we incidentally receive in the course of coordination with the confidentiality required of a business associate, and we contract accordingly where applicable.
Contact information — name, role, facility or organization, telephone number, email address, and any message content you provide via our contact form, phone, email, or SMS.
Service request information — facility name, request type (urgent, scheduled, discharge, transfer, etc.), timing, and any clinical or logistical context you provide to enable coordination.
Resident-related information — when a facility submits a transport request, we may receive limited information about a resident (initials, location, acuity, destination, mobility/equipment needs). We instruct facility partners to share only the minimum necessary information.
SMS consent records — if you opt in to text messages, we record the date, time, IP address (where available), phone number, and the specific language of the consent shown to you.
Usage data — IP address, browser type, device type, operating system, referring URL, pages viewed, and timestamps.
Cookies and similar technologies — small files that help our website function and allow us to understand aggregate usage. You may disable cookies in your browser settings; some site features may not work as a result.
We may receive information about you from transport providers, hospitals, facility administrators, or other parties involved in coordinating a transport, strictly for the purpose of completing the service requested.

Maintain operational records, including trip logs and utilization summaries provided to facility partners;

Comply with legal obligations, court orders, and lawful requests by public authorities; and
Protect the rights, property, or safety of Larkin ALF Ambulance, our partners, residents, or the public.
If you opt in to receive SMS messages from Larkin ALF Ambulance Service, the following terms apply:
Consent. You must affirmatively check the SMS opt-in box on our contact form (or otherwise provide express written consent) before we send you text messages. Consent is not a condition of receiving our services or making a purchase.
Message types. Messages may include dispatch confirmations, ETA updates, transport status notifications, appointment reminders, account-related information, and partnership communications. We do not sell SMS-collected phone numbers, nor do we share them with third parties for marketing purposes.
Frequency. Message frequency varies based on your activity and service usage.

Supported carriers. Major U.S. carriers including AT&T, T-Mobile, Verizon, and others. Carriers are not liable for delayed or undelivered messages.
SMS data sharing. Phone numbers collected through SMS opt-in are used only by Larkin Community Hospital, INC and its authorized message-delivery vendor (such as a licensed CPaaS provider) for the purpose of delivering the messages you consented to. Mobile information will not be shared with third parties or affiliates for marketing or promotional purposes.
We share information only as necessary to operate the service and comply with the law. Specifically:
Transport providers. We share the minimum information necessary (typically pickup location, destination, level of service, equipment needs, and resident identifiers as required) with the ambulance or transport provider we assign to a trip.
Receiving facilities. When we pre-notify a hospital, clinic, or receiving facility of an inbound transport, we share only the information needed for them to prepare.
Service vendors. Hosting, communications (SMS, email, telephony), analytics, and similar vendors that process data on our behalf under written agreements that restrict their use of the data.
Legal compliance. When required by law, subpoena, court order, or to defend our legal rights.
Business transfers. In the event of a merger, acquisition, or asset sale, information may be transferred subject to confidentiality protections.
We do not sell personal information. We do not share personal information with third parties for their own direct marketing purposes.
Larkin ALF Ambulance Service may, in the course of coordinating transport, receive or transmit information that constitutes Protected Health Information (PHI) under the Health Insurance Portability and Accountability Act of 1996 (HIPAA). Where we operate as a business associate of a HIPAA-covered entity, we execute a Business Associate Agreement (BAA) with that entity and handle PHI in accordance with its terms and the HIPAA Privacy and Security Rules. We apply administrative, physical, and technical safeguards designed to protect PHI consistent with industry standards.
Florida residents have certain rights under the Florida Digital Bill of Rights and other applicable Florida law, including, depending on applicability:
The right to confirm whether we process your personal information and to access that information;
The right to request correction of inaccurate personal information;
The right to request deletion of personal information we have collected;
The right to opt out of the sale of personal information (we do not sell personal information) and out of targeted advertising;
The right to opt out of profiling that produces legal or similarly significant effects;
The right to data portability where applicable.

We retain personal information only as long as necessary to fulfill the purposes for which it was collected, to comply with our legal, accounting, or reporting obligations, or to resolve disputes. Transport coordination records may be retained for longer periods where required by Florida medical or business record retention laws. SMS consent records are retained for at least the duration required to demonstrate compliance with the TCPA and applicable carrier requirements.
We maintain reasonable administrative, physical, and technical safeguards designed to protect information against unauthorized access, alteration, disclosure, or destruction. However, no method of transmission over the internet or method of electronic storage is 100% secure. We cannot guarantee absolute security and encourage you not to transmit sensitive information (such as Social Security numbers or full clinical records) through our website forms.
Our website and services are not directed to children under the age of 13. We do not knowingly collect personal information from children under 13. If you believe we have inadvertently collected such information, please contact us and we will delete it promptly.
Our website may contain links to third-party websites or services. We are not responsible for the privacy practices or content of those third parties. We encourage you to review their privacy policies before submitting information to them.
